- Birth Certificate from the National Population Commission or Affidavit of Declaration of Age
- Valid means of identity such as Valid Driver’s License, National ID Card, National Identification Number (NIN), Nigerian International Passport, or Voter’s card
- Printout of the response to your online application.
- Recent Passport photographs
- Certificate of Local Government Area of Origin (Local Government Identification)
- Printout of your registration slip
